A North Carolina hospice recently began an initiative to encourage people to ‘begin the conversation’ about communicating healthcare choices. You can begin the conversation with your loved ones by downloading a free toolkit here. The toolkit includes worksheets with questions such as:
If you couldn’t speak for yourself, who would you want to represent your wishes and make sure they are carried out?
Have you identified the treatments you would want to receive or refuse if you are injured or ill?
Are there things you want to say to your loved ones now while you have the time?
