Hospice Admission

How Do I Begin Hospice Services?

Hospice of the Comforter accepts referrals from physicians. However, friends, family or the patient may make the initial call to Hospice of the Comforter to discuss whether or not a patient is appropriate for hospice services. If he or she is, Hospice of the Comforter will then work with the patient’s physician to obtain the referral.

To make a referral, call 407-682-9090, 24 hours a day, 7 days a week.

What to Expect When You Call Our
Referrals and Admissions Department

A Referral Nurse will answer your call. He or she may ask you these questions:

  • What is the patient’s diagnosis?

  • What is the patient’s current condition?

  • Who is the patient’s primary physician?

  • Who is referring the patient to hospice?

  • What type of insurance coverage does the patient have? Medicare? Medicaid? Other?

The Referral Nurse will then speak with you and answer any questions, including those about:

  • Palliative (comfort) care, symptom and pain control

  • Scheduling a visit to your home, skilled nursing facility, assisted living facility or hospital

  • Hospice of the Comforter’s services

To make a referral, call 407-682-9090, 24 hours a day, 7 days a week.
