As part of the 2008 Strategic Planning Goal(s), HOTC is charged with accomplishing the following: Provide the Highest Quality Employee Care (as evidenced by . . .)

A competitive compensation structure to recruit and retain the highest quality employees

Rationale: A strong and well-defined compensation structure allows the organization to recruit and retain the highest quality employees. This is essential in an increasingly competitive market to sustain the health of the organization and to support providing the highest quality patient care.

Goals of this project are:

  • To ensure that we establish fair, consistent rates of base pay for all jobs within the organization based on the duties and responsibilities
  • The market value of such work.  

To successfully accomplish this goal we need your help!  We are requesting that you complete the attached job analysis questionnaire of your job at this time

The job questionnaire is available for completion in two forms, electronic or paper. To minimize the cost of this project, we are asking that every person able to complete the job questionnaire electronically please do so.  The electronic format conveniently allows you to complete sections of the job questionnaire as your time permits.   See Frequently Asked Question List below for procedures on opening and saving the questionnaire.

All job questionnaires need to be returned via email to Riana Hurst, HR Assistant rianahurst@hospiceofthecomforter.org or to the HR “Job Questionnaire” mail folder in the 1st floor copy room of the 480 W. Central Parkway Building by April 14, 2008.

As HR receives the completed job questionnaires, all employee identifiers such as your employee ID number and your email address (if applicable) will be removed and the job questionnaires will be compiled as a group for your department head to review.

Remember – if you have questions or concerns regarding the Job Questionnaire see Lynn Wollin, HR Director or Amanda Dowd, HR Manager as soon as possible.

Frequently Asked Questions

Q:        Why did HOTC ask me to complete the job questionnaire when I have the same job as other people in this organization?

A:         We have requested that every employee complete a job questionnaire to allow every employee the opportunity to provide input on the job they do every day.  Remember the questionnaire is asking questions about the job, not your resume or your skill set.

 

Q:        Where can I find an electronic version of the job questionnaire?

A:         An electronic version is available two ways, 1) as an attachment through an email from your manager or the HR Assistant, Riana Hurst, or 2) a download from the HOTC intranet UltiPro Web.

 

Q:        If I complete the questionnaire electronically, can I start and stop working on the job questionnaire at any time?

A:         Yes, as long as you have followed the steps to save the document, you can complete portions of the questionnaire as your time permits.

 

Q:        Will my name, email address and employee ID number be removed from the job questionnaire before it is sent to my manager?

A:         Yes, every employee’s name, employee ID number and email address will be removed from the job questionnaire by the HR Department.  Then all job questionnaires will be compiled as a group and distributed to each manager.

 

Q:        If I work Fee for Service which job do I complete the Job Questionnaire for?

A:         Complete the questionnaire for your primary job here at HOTC, if you would have time you can also complete a second questionnaire for the job you work Fee for Service.

 

Q:        How will information regarding the project be communicated with me?

A:         It is the intention of HOTC management to keep you informed of the progress of the project.  At the conclusion of the project information related to your job will be shared with you (e.g. completed job description and salary range).

 

Q:        How do I complete the job questionnaire electronically?

A:         1. Open the appropriate attached Word Document above.

            2. To identify the questionnaire, Select File, Save As a new document using your employee ID Number.  For example, Questionnaire – 1291

            Saving the document will allow you to complete portions of the questionnaire and return to the questionnaire as your time permits.

            Once you have completed the Job Questionnaire

            3.         Send a new email message addressed to Riana Hurst, HR Assistant at                        rianahurst@hospiceofthecomforter.org

            4.         Attach the questionnaire, using the paper clip symbol and click Send
